A Specialist Step-Down Pathway from Hospital to Community Living

Our Residential Care with Nursing service provides a specialist step-down pathway for young people with complex mental health needs who have experienced prolonged hospital stays, placement breakdowns, or require intensive therapeutic support following discharge.

Designed for individuals aged 13–18, our service enables safe transition from inpatient settings into community-based homes, where specialist nurse-led care supports stabilisation, recovery, and long-term progression towards independent living.

Each young person lives in a single-occupancy home within a normal residential community, creating a structured yet nurturing environment where clinical support, therapeutic input, and daily living skills development can take place outside of institutional settings.

Who This Service Is For

Our residential care pathway supports young people with complex needs who are unable to transition directly from hospital into traditional community placements.

Typical referrals include:
Long-stay acute mental health patients
Forensic or secure step-down placements
Young people with complex trauma
Individuals who have experienced multiple placement or package breakdowns
Patients transitioning from hospital via Section 17 leave
Young people requiring Section 117 aftercare
Referrals are typically received from Local Authorities, Integrated Care Boards (ICBs), CAMHS teams, and hospital discharge planners seeking safe alternatives to long-term inpatient care.

Nurse-Led Therapeutic Care

Care is delivered through intensive, clinically supported packages tailored to each young person’s needs. 

Most placements begin with 3:2:1 care packages, typically consisting of:
Registered Mental Health Nurse (RMN)
Healthcare Support Workers (HCAs)
24-hour supervision and support
This structure ensures safe stabilisation following discharge from hospital or placement breakdown.

Our teams prioritise mental health recovery and emotional stability, working closely with multidisciplinary professionals, including:
CAMHS teams
Social workers
Behaviour specialists and Positive Behaviour Support (PBS) practitioners
Clinical therapists and other specialist professionals

Stabilisation and Recovery

Many of the young people referred to our service have experienced significant disruption within previous placements. Our homes are designed to provide consistency, safety, and therapeutic stability, allowing individuals to rebuild trust, confidence, and emotional regulation.

Within this environment, we support:

As individuals begin to stabilise, care packages are gradually reduced — supporting increased independence while maintaining appropriate safeguarding and support.

Residential Care vs Supported Living

Our residential care service represents the first stage in a longer-term pathway towards independence.

Residential care provides intensive clinical oversight and therapeutic support, often following hospital discharge or placement breakdown. Staffing levels are higher and care packages are designed to stabilise complex mental health needs.

Supported living focuses on promoting independence, with individuals living in their own accommodation while receiving flexible support from care teams.

Our model allows individuals to move progressively through levels of support.

This pathway ensures that care evolves alongside the individual’s recovery, supporting safe and sustainable community integration.

Community-Based Homes

All of our residential placements are delivered in single-occupancy homes within normal residential communities.

This approach helps young people experience a more natural living environment while still benefiting from intensive clinical support and therapeutic structure.

The tenancy for each property is held and managed by our organisation, ensuring stability for young people who may not yet have the capacity or legal status to hold their own tenancy.
