Express Care at Home has Nurse-led governance at it’s core, ensuring all complex packages are clinically overseen with robust risk assessments, structured care planning, regular clinical review and clear escalation pathways. We prioritise safety, stability and continuity. Where required, we can mobilise urgent care packages within 24 hours (subject to assessment and location). We understand the pressures of discharge timelines and funding approvals and work proactively to avoid delays.
