About the role

Express Care at Home are a leading provider of complex care services, delivering specialist support to clients with high-dependency needs across Yorkshire, the North West, and the North East. We work closely with Integrated Care Boards (ICBs), Local Authorities, case management companies, and allied health professionals to ensure outstanding, person-centered care.

We are looking for a Senior Care Coordinator with extensive experience in complex care and a proven track record of managing high-value care packages. This role is pivotal in building and maintaining strong relationships with ICBs, Local Authorities, and other stakeholders, while ensuring that every client receives safe, high-quality, and tailored care.

The ideal candidate will have at least 5 years’ experience in complex care, with in-depth knowledge of care commissioning, regulatory frameworks, and the nuances of working across multiple sectors.

Key Responsibilities

Key Responsibilities:

Care Coordination & Oversight

  • Lead and manage complex care packages, ensuring excellent outcomes for clients with high dependency needs.
  • Develop, implement, and review person-centered care plans in line with statutory guidance and best practice standards.
  • Conduct risk assessments and proactively manage any challenges in care delivery.

Stakeholder Engagement & Relationship Management

  • Act as the primary liaison with ICBs, Local Authorities, case management teams, and other key partners.
  • Build strong, long-term professional relationships to support service expansion and high-quality care delivery.
  • Represent the organisation in meetings, reviews, and audits with commissioners and regulators.

Team Leadership & Development

  • Provide mentorship, guidance, and supervision to junior coordinators and care staff.
  • Support professional development and encourage continuous improvement across the team.
  • Lead initiatives to improve service quality, operational efficiency, and client satisfaction.

Strategic Contribution

  • Identify opportunities to grow services within existing and new contracts.
  • Collaborate with senior management to inform business strategy and service planning.
  • Contribute to policy and procedure development, ensuring compliance with all statutory and contractual obligations.

Candidate Requirements

  • Minimum 5 years’ experience in complex care, preferably within ICB and Local Authority frameworks.
  • Strong knowledge of adult social care regulations, safeguarding, and commissioning processes.
  • Excellent stakeholder management skills with experience influencing commissioners and senior decision-makers.
  • Proven leadership and team development skills.
  • Highly organized, with the ability to manage multiple high-value cases simultaneously.
  • Strong analytical, communication, and negotiation skills.
  • IT proficient, confident in care management systems and Microsoft Office applications.
  • Full driving license with access to a vehicle for regional travel.
